by you. We will also need a copy of your original prescription.

No other information
is needed unless
there has been

address, health
condition, in
the strengths
or quantities
of the medications
you are taking,
or in the method
of payment. Once
a prescription
has expired,

to provide a
copy of the new
prescription
issued by your
doctor.
