Sign the order form and FAX it to us (toll free) 1-877-278-5359. Don't forget to fax a copy of the original prescription from your doctor.

Once we receive your order we will confirm your order by Email. In your order confirmation we will provide account information so you can track your order online. The prescription will be sent to the address on your order form.
